Intellect NUS Disclaimer

All sessions conducted with Intellect providers are confidential in perpetuity. Confidential information may be disclosed to other relevant  personnel and third parties without first obtaining my expressed consent in certain situations, including, without limitation, the following :  

Exceptions to Confidentiality

These are scenarios where your Provider may need to break confidentiality by informing other parties about your care or share information about your person, or is obliged to do so by the law:

  1. Failure to disclose the information would place you or another person at risk of harm.
    1. cases in which the purpose of such disclosure is clearly in my interests, and if consent cannot be obtained in a timely way provided that Intellect, as soon as, may be practicable, notify me of the disclosure and the purposes of the disclosure
    2. assessment and interventions related to my concerns are to be conducted by other health professionals; 
  2. If a disclosure of facts/incident requires Intellect to submit a report to the necessary regulatory or law enforcement authorities.
  3. When your information is subpoenaed by a court and such disclosure  is otherwise required by law
  4. I have authorized such release in writing or by verbal permission.

1) Cases in which the purpose of such disclosure is clearly in my interests, and if consent cannot be obtained in a timely way;

2) Cases in which the disclosure is necessary to respond to an emergency that threatens the life, health or safety of myself or another individual;

3) Cases in which there are reasonable grounds to believe that the health or safety of myself or another individual will be seriously affected and consent for the disclosure of the information cannot be obtained in a timely way, provided that the University Counselling Services and/or the University Health Centre of NUS shall, as soon as may be practicable, notify me of the disclosure and the purposes of the disclosure;

 

Your Provider will discuss (whenever possible) the matter with you before breaking confidentiality. 

 

Helpline calls received by Intellect will be reported to University Counselling Centre ( UCS), NUS by the next working day. UCS may reach out to the caller where needed.

 

By continuing, I understand the nature and limits of confidentiality, and what is expected of me as a client of Intellect Services. I agree to indemnify the Company and/or the Provider from any and all actual or alleged claims, demands, causes of action, liability, and/or injury (to property or persons, including without limitation wrongful death), whether brought by me, any individual, or other entity.
